The transition from the entitlements of the school system to the eligibility for adult services can be confusing. Join us to learn more about the necessary steps you can take to help prepare for this transition and actions you can take to ensure that your child has the necessary supports as they exit the school system at 21yrs.
Ebony Steele, Support Coordinator and Coordinated Care of MetroWest Case Manager for JFS MetroWest, will share her expertise from working with individuals, families and support coordinators about the transition process.
Workshop highlight will include:

• Who should be included in the transition planning process?
• Eligibility vs Entitlement
• Applying for Medicaid, SSI and DDD
• What is the NJCAT?
• Choosing a Support Coordinator
