Navigating Support Coordination With Confidence

What Parents & Caregivers Should Know

Support Coordination is a key resource for individuals and families that helps plan for and guide all adult services for an individual eligible for DDD services.   A Support Coordinator will work with the individual and their family to begin the person-centered planning process, including the Individualized Service Plan (ISP).

Ebony Steele, Support Coordinator and Coordinated Care of MetroWest Case Manager for JFS MetroWest, will share her expertise about the process of identifying a support coordinator, their roles and responsibilities and explaining the different waivers, tiers and acuity.

Workshop highlight will include:

  • What options are available for my young adult transitioning to adult services?
  • Person Centered Planning
  • ISP – Individualized Service Plan
  • Waivers, Tiers and Acuity – What do they mean?

This informative workshop will provide a wealth of knowledge for parents and caregivers as they prepare to partner with a Support Coordinator.

Security at Federation Events

The Gloria and Jesse Weissberg Community Security Initiative (CSI) is coordinating all security with local, county, and state law enforcement. At every Federation event, all available resources are utilized to ensure your safety. Important items of note:

  • Bag policy: Backpacks, luggage, and large purses are not permitted at events. We encourage you not to bring a bag, if possible, or to bring a very small bag. All bags may be subject to search.
  • Additional security measures: Additional security procedures may be implemented (e.g. metal detectors) as needed.
  • Social media protocol: We ask you not to share the location of a Federation event prior to or during the event and to refrain from posting about a Federation activity until after it has ended, and the site is clear of attendees.
