Transitioning to Adulthood – Preparing for Age 21: What Parents & Caregivers Should Know

The shift from the entitlements of the school system to the eligibility requirements for adult services can feel overwhelming. This workshop will guide you through the necessary steps to prepare for this transition and ensure your child has the supports they need as they exit the school system at age 21.

Join Ebony Steele, Support Coordinator and Coordinated Care of MetroWest Case Manager for Jewish Family Service (JFS) MetroWest, and Denise Rekem, attorney at Parles Rekem LLP, as they share their expertise in guiding individuals, families, and support coordinators navigate this important process.

Workshop highlights include:

  • When to begin transition planning
  • Key participants in the planning process
  • Understanding eligibility vs. entitlement
  • Applying for Medicaid, Supplemental Security Income (SSI), and Division of Developmental Disabilities (DDD) services
  • What is the New Jersey Comprehensive Assessment Tool (NJCAT) and why does it matter?
  • How to choose the right Support Coordinator

This essential workshop will equip parents and caregivers with the knowledge and tools to confidently prepare for their child’s transition to adulthood.

Security at Federation Events

The Gloria and Jesse Weissberg Community Security Initiative (CSI) is coordinating all security with local, county, and state law enforcement. At every Federation event, all available resources are utilized to ensure your safety. Important items of note:

  • Bag policy: Backpacks, luggage, and large purses are not permitted at events. We encourage you not to bring a bag, if possible, or to bring a very small bag. All bags may be subject to search.
  • Additional security measures: Additional security procedures may be implemented (e.g. metal detectors) as needed.
  • Social media protocol: We ask you not to share the location of a Federation event prior to or during the event and to refrain from posting about a Federation activity until after it has ended, and the site is clear of attendees.
